What Phase 2 Showed
In the NEJM Phase 2 program (NCT05669599):
Obesity without T2D (efficacy-style public summaries): up to about ~20% mean weight loss at 52 weeks vs ~2–3% placebo, without a clear plateau.
Obesity with T2D: up to about ~17% mean weight loss, with HbA1c reductions up to roughly 2.2 percentage points in Amgen communications.
Treatment-policy / intention-to-treat style means in the NEJM report were somewhat lower (roughly low-to-mid teens % in the obesity cohort across active arms) — always check which estimand a headline uses.
Why Escalation Exists
GI adverse events were common. Public summaries note they were less frequent with dose escalation and lower starting doses. That is the same titration philosophy as weekly incretins — just on a monthly clock.
